HEROES KIDS SUMMER CAMP 2026 — PARENTAL CONSENT & WAIVER AGREEMENT
By registering my child/children for the Heroes Kids Summer Camp (HKSC) 2026, I, the undersigned parent or legal guardian, hereby acknowledge and agree to the following terms:
1. Physical Activity & Injury Risk
I understand that this camp involves combat sports, physical training, and martial arts activities. I acknowledge that there is an inherent risk of physical injury, and I voluntarily accept this risk on behalf of my child. I confirm that my child is physically fit to participate and has no medical conditions that would prevent participation without prior written medical clearance.
2. Emergency Medical Treatment
In the event of an injury or medical emergency, I authorise the camp organisers and staff to seek and administer emergency medical treatment on behalf of my child. I agree to be responsible for any associated medical costs not covered by the camp.
3. Photography & Media Use
I grant ATC Global Sports Tours and The Heroes Gym permission to photograph and/or video record my child during camp activities. These images/videos may be used for promotional, educational, and social media purposes without compensation.
4. Code of Conduct
I agree to ensure my child adheres to the camp's code of conduct, which includes respect for coaches, fellow participants, and equipment. I understand that any serious misconduct may result in removal from the camp without refund.
5. Data Privacy
I consent to ATC Global Sports Tours collecting and storing the personal information provided in this form for the purposes of camp administration, communication, and record-keeping, in accordance with applicable data protection laws. This data will not be sold or shared with third parties without consent.
